Coexistence of aldosterone-producing adrenocortical adenoma and pheochromocytoma in an ipsilateral adrenal gland.

چکیده
A 40-year-old female, diagnosed as essential hypertension, demonstrated a 2 cm mass in left adrenal gland by computed tomography without abnormal endocrinological findings. (131)I-adosterol and (123)I-metaiodobenzylguanidine (MIBG) scintigraphy at 39 years of age showed no abnormal accumulation. Follow up (131)I-adosterol scintigraphy performed one year later showed apparently abnormal uptake and slightly elevated uptake in left adrenal gland. Her physical examination was unremarkable except for mild hypertension. Routine blood chemistry was normal except for hypokalemia. Endocrinological date revealed suppressed plasma renin activity, and elevated plasma aldosterone concentration, and noradrenalin levels. Serial T2-weighted magnetic resonance imaging clearly demonstrated two distinct tumors. Furthermore, selective adrenal venous sampling with intravenous ACTH infusion indicated aldosterone-producing adrenocortical adenoma (APA) in left adrenal gland. During operation of adrenal tumor, blood pressure elevated markedly and complication of pheochromocytoma (PC) was suspected. Immunohistochemical findings after left adrenolectomy revealed that the adrenal mass was compatible with APA and PC. Risk of operation against undiagnosed PC is very high and, therefore, it must be diagnosed before surgery. Herein, we present an extremely rare case of the simultaneous occurrence of both APA and PC in an ipsilateral adrenal gland.

Adrenocortical Carcinoma Mimicking Pheochromocytoma: A case Report and Review of Literature
Adrenocortical carcinoma (ACC) is an uncommon malignancy originating from cortex of adrenal gland. The most common pitfall in diagnosis of ACC is to distinguish it from pheochromocytoma. Here we report a 62-year-old hypertensive man with presentation of dyspnea. The laboratory data showed an increase in urine cortisol and renin with a mild increase in aldosterone, but decrease in adrenocorticot...
متن کاملTwo tumours of the right adrenal gland treated videoscopically by the posterior retroperitoneal approach
Pheochromocytomas are a rare cause of arterial hypertension, necessitating surgical resection. Coexistence of a pheochromocytoma with an ipsilateral adenoma in the right adrenal gland has sporadically been reported. No reports on their videoscopic resection by the posterior retroperitoneal approach are available. We describe a case of a 49-year old female patient with a pheochromocytoma of the ...
متن کاملAdiponectin receptor expression in the human adrenal cortex and aldosterone-producing adenomas.
Adiponectin is an adipocyte-derived circulating peptide that plays an important role in adipose tissue metabolism, insulin sensitivity and cardiovascular disease. The adrenal gland, by secreting glucorticoid and mineralocorticoid hormones, intervenes in cardiovascular and glucose metabolism regulation and is surrounded by adipose tissue. Hence, we investigated the hypothesis that adiponectin re...
متن کاملPrimary aldosteronism in a patient with an aldosterone-producing adenoma.
We describe an unusual patient presenting with a history of refractory hypertension and hypokalemia. Initial screening tests for adrenal hypertension were consistent with primary aldosteronism and an abdominal computed tomography scan showed an 8-mm left adrenal mass. However, adrenal venous sampling revealed markedly suppressed plasma aldosterone in the left adrenal vein but increased plasma a...
متن کاملAnalysis of mRNA expression for steroidogenic enzymes in the remaining adrenal cortices attached to adrenocortical adenomas.
DESIGN AND METHODS We have recently demonstrated that the adrenal cortices attached to aldosterone-producing adenoma (APA) contained microscopic subcapsular micronodules suggestive of active aldosterone production. In this study, we used in situ hybridization to investigate the mRNA expression of steroidogenic enzymes in the adrenal cortices attached to cortisol-producing adenoma (CPA) and clin...
